Learn about running the Cancer Transitions program in your facility. A critical component of Cancer Transitions is the facilitation by cancer care professionals. 

If you have a Cancer Support Community, it is easy to work with them and offer the Cancer Transitions program. If you don’t, Cancer Support Community offers resources and training for program delivery. 

Resources that accompany the program include and online Train-the-Trainer program developed for oncology professionals who will run the program in their office/facility.  Cancer Support Community will also provide additional training support for oncology professionals running the program.  More information about the Train-the-Trainer program will be coming soon.
